在宅療養者とその家族・関係スタッフにおける要食事介護者向けアイスクリーム活用可能性についての検討

Abstract

アイスクリームは食欲不振時にも食べやすく，低栄養の改善にも使われるが，溶けやすい性質から嚥下機能低下や喫食に時間を要する場合は提供が難しい。そこで，考案した要食事介護者向けアイスクリームが介護食として活用できるかの可能性を検討するため，訪問栄養食事指導を利用する在宅療養者とその家族・関係スタッフを対象にアイスクリーム喫食後質問紙調査を行い，コレスポンデンス分析ならびにCS分析を行った。在宅療養者26名と家族・関係スタッフ31名の回答結果より，「食べ終わりまでアイスクリームとして楽しめたか」の質問に対して「十分に楽しめた」「おおむねアイスクリームとして楽しめた」の回答を合わせると，在宅療養者は96.1％，家族・関係スタッフは87.5％であった。コレスポンデンス分析では「総合的に判断して，今後食べたいか」の項目で「嚥下機能維持」は「あれば食べたい」，「嚥下機能低下」は「ぜひ食べたい」「食べたい」と近くに分布しており，自由記述からも嚥下機能低下群において本アイスクリームに対する需要性が示唆された。一方，CS分析の結果では，アイスクリームの提供量と風味・甘さについて改善度が高かった。しかし，家族・関係スタッフ31名中30名が「総合的に判断して，介護食として利用できる」と回答しており，自由記述からも食介護の現場において，本アイスクリームに対する需要性と活用の可能性が見いだせた。Ice cream is easy to eat even when appetite is lost and is used to improve undernutrition. However, it is difficult to serve ice cream dysphagia or when it takes time to eat due to its dissolving nature. Therefore, in order to examine the possibility of whether the devised ice cream for caregivers requiring meals can be used as meal care, we conducted a questionnaire survey after eating ice cream among patients who use home-visit nutritional dietary guidance, their families, and care staff, and conducted correspondence analysis and Customer Satisfaction (CS) analysis. From the results of the responses of 26 home care patients and 31 family and care staff, the total of the answers “enough” and “generally” to the question “Did you enjoy it as ice cream until you finished eating it?” was 96.1% for home care patients and 87.5% for family members and related staff. Correspondence analysis showed that the dysphagia group was related to “definitely want to eat” and “want to eat”, and that the open-ended statement also showed a demand for this ice cream in the dysphagia group. The results of CS analysis showed that the amount of ice cream served, flavor and sweetness were improved points. However, 30 out of 31 family members and related staff responded that it could be used as a nursing care food based on a comprehensive judgment, and it suggested the demand and possibility of this ice cream in the field of nursing care.
