「介護福祉実践」事象をめぐる論争 : 1990年代後半－2000年代

Abstract

介護福祉士創設（1987年）から30年になる。この間の介護福祉実践の性格についての議論を 1）介護福祉士創設直後の社会福祉の視点が強調された期間（1990年代前半まで），2）介護福祉士業務における「医療」をめぐる議論が起きた時期（1990年後半～2010年），3）「医療的ケア」導入によって介護福祉士と看護師の一応の「職域分担」が明示された2011年以後）の3つに分けられる。2）の時期の議論には，3つの立場があった。第一は「看護の立場」，第二は「社会福祉の立場」，第三は「第三の立場」であった。2011年の「医療的ケア」導入後，看護師と介護福祉士の「棲み分け」がなされ，両者の連携が強調されるようになる。「看護の立場」論は弱くなった。しかし，「看護の立場」論の代表的な研究者髙木は，「日常生活の営み」支援＝「世話」（看護）を展開し，「第三の立場」論に大きな影響を与える。本稿は介護福祉実践の事象を，制度上また，先進国の長期ケアの流れの中での日本の介護福祉実践の位置を確認し，2）の時期に「看護の立場」を展開した髙木の言説（「日常生活の営み」支援＝看護）を振り返り，介護福祉学構築の視角を定めることにある。 Thirty years have passed since the establishment of the Japan' certified care worker system in 1987. Debate regarding the implementation of care work over this time can be divided into the following three periods: 1) The period of establishment of care worker certification (until early 1990s), 2) the period during which debate regarding “medical care” in care worker duties (late 1990s–2010) and 3) the period from the introduction of “medicalized care” onward (2011 onward). Debate regarding 2) was based on three standpoints—the nursing standpoint, the social welfare standpoint and the third standpoint. The introduction of “medicalized care” in 2011 marked distinguished roles of nurses and care workers, resulting in increased emphasis on inter-professional collaboration. Theories on the standpoint of nursing became weaker. However, Takagi, a renowned researcher on nursing standpoint theory, developed the notion that “everyday life” support=“care” (nursing), which greatly influenced “third standpoint” theory. This paper investigates the implementation of care work, reviews Takagi's view that “everyday life” care=nursing, which was developed based on the “nursing standpoint” in the period described in 2), and lays out the perspective and groundwork of care work.
